Any library district patron may sign of for this program:
• who, for physical reason, has been determined to be temporarily or permanently homebound may sign up and
• without able-bodied family or live-in help to assist with delivery.

How does it work?
Begin by filling out the application form. There are two ways this program can assist you:
• library staff will collect library materials for you, and a family member, neighbor, friend or caregiver will pick up the materials for you at the library's front desk.
• library materials will be delivered to your door on a monthly schedule.

How do I sign up?
Sign up by filling out the application form. Make sure that your library card is current, and then drop off or mail this form to set up service.

What if I don't have a library card?
If you do not have a library card and live within the Cherry Valley Library District, you may obtain one during all operating hours at the library's circulation desk. If necessary, a library volunteer can come to you to gather the needed information.

What library books and media can I receive?
Any items that can be checked out at the library can be requested for the Homebound bookbag. There is a limit of 10 items at one time.

What is the frequency of requesting and receiving library items?
The library will select library items once a month on a schedule agreed to by you and your designated family member, neighbor, friend, or caregiver, OR a library volunteer.

Do I have to be at home for homebound volunteer delivery?
Yes. If you are not going to be home, please contact the library prior to your scheduled delivery time. Your requested materials will wait until a new time that a volunteer can accommodate your needs.
